Acute Rheumatic Fever as Child & Adult symptoms

I had ARF when I was a child of 4 and now am almost 54. The treatment at the time was to administer penicillun every day until I was 14 (or 18 ?).
For the past 6 years I have had symptoms that seem to baffle medical professionals from conventional as well as alternative and I am wondering if they are related to this early disease and treatment.
My first symptoms were swelling, burning and itching elbows. Sometimes this would include my shoulders as well. Additionally, I had a horrendous rectal itch and scalp itch at the base of my scalp to my ears. Inside my ears itched as well. Then I had frozen shoulder in my left shoulder, my eyelashes fell out and my front hair started thinning terribly. Still, I had no relief from these symptoms with an extreme sugar free, all organic diet. I did 3 months of an herbal parasite cleanse and many of the symptoms reduced. A dermatologist then gave me a strong anit-fungal to take for 7 weeks which rid me of the scalp and ear itch. I felt wonderful for a year. Then I was involved in a car accident and didn't exercise much for 3 months and my eating habits weren't the best although still quite good comparatively. Then all these previous symptoms returned including frozen shoulder now on my right side. In addition, I now have a thyroid disease (slightly hypothyroid with antibodies that attack my thyroid) so I take daily thyroid medication. It improves my energy a little and slowed down weight gain. I go to the gym 5 days a week, I am in good physical condition, and have an excellent diet 90% of the time eating mostly organic foods. I had a procedure for my right shoulder called manipulation under anesthesia rather than surgery. It has given me only about 50% back and I still experience pain and numbness in the morning. I do not have the rectal itch but do continue to have the red burning swelling elbows and the back of my head scalp condition they label as dermatitus. My eyelashes are still minimal and my hair thin. Doctors have a hard time thinking I have anything wrong with me as I look like I am in my late 30's. I have always had that 'young' look and positive outlook on life which is deceiving to them. It would be nice to get a definitive diagnosis so I could deal with this chronic condition and be done with it. Any ideas out there?
