What are some of the causes of bad breath or odor being expelled through the breath?

I guess I've had bad breath or odor being expelled orally longer than I realize. For years, I was misdiagnosed as having chronic colds when it was my sinus. I have since had my tonsils, adenoids, uvula and other thing ear/nose/throat related removed hoping to eliminate the odor but it seems to persist. Sometimes, individuals standing 1-2 feet away can notice it but I don't smell anything. My dentist also has given me a clean bill of health. I have also noticed one of my children having bad breath but his oral hygiene is exceptional.

Can anyone help or guide me to someone who can?
