Difficulty taking deep breaths and sharp pain in left side of ribcage

I've been having difficulty breathing for the past 3-4 months. It feels like I am almost breathing through a straw, it is very difficult to take deep breaths. I've been smokeing for about 3 1/2 years, about a pack-pack and a half a week. I had a chest xray and was put on an inhaler. The xray was normal and the inhaler doesnt work.

Also, just recently now i am experienceing a sharp pain in the left side of my ribcage. It feels like there is a lot of pressure (only when i breathe) and it feels like if i made an incision or put pressure on my ribs (where the pain is) it would relieve the pain.

What could this be and/or what tests should I get next?
