breathing problems when exercising

i have breathing problems when i run for long periods of time or even walking a long time.I will have a burning pain in the base of my throat,headaches,swelling or burning of the tongue ,slightly dizzy,coughing ,wheezing,chest tightening and it's hurts to breath.It lasts at least an hour it gets worse when it's cold outside.I was born prematurely and had breathing problems early on.This has been happening for many years it's recently gotten worse.i have had pneumonia 5 years ago and I'm 13 .People thinking I'm lying or making this up or they assume i have asthma (which i don't have) i don't what it is and I'm scared if i have to do rigorous exercise for a long time i could blackout or even die.Help me!
