Hot burning Feet and Hands

I have had this problem for 4 years. This usually happens at night when I drift off into a rem sleep, the burning jolts me awake, I do the fan, wet towel, bucket of water and that only works sometimes, Been to the doc for several years, he has tried every drug known to man. The symptoms escalate with drinking of alcohol, esp wine, spicy food, general tiredness which is all the time, I do consume a fair amount of salt so will try dropping that out. I am very fit healthy and low BMI, The pain affects my feet, especially around the bunion area at night. I have some kind of circulatory problem, half my fingers turn transparent/white if Im cold or just after excercising when my body temp drops and have no feeling in them. I took a photo and showed the doc which he was quite shocked. If anybody has any ideas on this set of problems I would like to hear, this just sux as sleep deprevation is soul destroying.
