Jaw Pain

I've had minor to extreme jaw pain for nearly three years. I believe its from bruxism or clenching my teeth while sleeping. It first started on the left side of my jaw then moved to the right. The pain used to inhibit me from eating. It stopped that eventually but now the pain is traveling up to around my right eye and my head. It also makes an awful noise that sounds like ripping muscle tissue. I asked my dentist for help about a year ago and he charged me $600 for a mouth guard that doesn't work.
If anyone has any tips on treatment or just small helpful ideas I would really appreciate that. I would like to know if anyone here is having a similar problem.
