elbow/shoulder/neck pain for 3 years

Hi, I have been suffering from pain especially in my elbows for a few years now. The pain also travels down into my forearm, hand and fingers on both sides. It has been diagnosed as tendonitis, tennis elbow, and cubital tunnel, but the tests such as emg, mri, bone scan, x-ray, have not supported these diagnoses to the extent that the doctors and surgeons may have liked. i'm wondering if this is a neck problem because i have pain in the right side of my neck, it seems to travel deep into my shoulder and down to my elbow. can anyone suggests what this might be? I also get a sudden twitch in my neck every couple of months that makes my neck ***k and it hurts a little.
