untreated Chronic Chest Pain

My 30 year old cousin has the following symptoms:

Chronic chest pains since his early teens,
It is aggravated by movemement and vigorous activity,
Pain in the upper back,
Pain in the joints,
General body weakness/fatigue,
Warm clothing causes a lot of discomfort; so he tends to favour light garments on his upper body/trunk.
He does not get relieve from taking analgesics.

He does not cough.

He has never gotten the right treatment so you can imagine he has had to live with the pain for over 15 years!

The X Ray pictures have picked nothing wrong.

Will you please help me get a diagnosis?
