Sharp pain near scrotum, difficulty urinating and urinary retention

Hi Everyone,

Really hoping someone can shed some light on what is wrong with me! Im a 19 year old male, healthy weight and generally healthwise ok.

About a month and a half ago i experienced difficulty urinating - i would have to push for anything to come out and maintain pushing for the duration. I also was experiencing VERY sharp needle like pains on the underside/back of my scrotum and down the tube towards my anus.
I went to my doctor, she examined me for prostatis, STI's and took urine tests which came back fine. She gave me a 5 day course of antibiotics incase it was a urinary tract infection.

Over the next week and a half the pain started to subside and gradually disapeared and i was able to urinate normally again.

However, a week and a half ago the symptoms have returned, i also have extreme constipation which wasn't present before even though im drinking plenty of water and have alot of fiber. I have a burning pain after ejaculation on the tip of my penis which lasts 5-10 seconds aswel which was present the last time i had this.

Im really desperate for some advice, so if you have any ideas please post?
