heart cath gone wrong

need advice on left heart cath,coronary angiography,left ventriculography,attempted stenting rca.-------revieled severe rca disease----while in process of attemting a pci of rca there was vessel closure,vessel dissection as well as,paroxysmal aortic dissection.patient transfered to trauma center-enroute suffered cardiac arrest requiring 30 to 40 minutes of cpr-patient revived and underwent emergent 1-single vessel bypass as well as aortic dissection repair.------how does this happen?
