Hard red skin patches

About once every 2 months, I get small pimple like hives that get itchier. By scratching them, they turn into sometimes 2 inches by one inch long hard red skin plates that are extremely itchy. Used to take cortizone pills that would usually treat it but now seem uneffective.... ?? can someone help?
