False positive for nordiazepam

I am baffled by this. Drug screens (blood and urine) have come back either "tainted" (unable to use) or they have found trace amounts of nordiazepam. I do not take any kind of benzodiazepine, and am upset that this could be happening. Is this the result of some type of inborn error of metabolism, metabolic disease or a result of improper collection techniques (although I fail to see how blood draws could be tampered with)?
