esophageal varices - unusual cause

I have 3 varicose veins in the esophagus, two of which have been banded. Everything I read suggests that the cause of esophageal varices is portal hypertension - but I don't have this. I have also had liver doppler, liver ultrasound CT scan of liver and liver biopsy - all are clear. What else can cause esophageal varices?
