Epigastric Pain with trouble swallowing

I need some advice my husband is experiencing constant epigastric pain with trouble swallowing, heart burn with no relief from antacids, N/V, when the pain is intense he has chest pain. His BP is 108/52 pulse is 54 and respirations is 18. His BP and pulse are low for him. No cough, or drainage,no headache, no diarrhea. No recent trauma to area. Feels like his throat is closing up when he drinks. Slight chest pain when takes deep breathe. On a pain scale he states its a 9. He's not a great a historian. If someone can please give me some advice so I can get him help I would greatly appreciate it.
