Swollen knee

A 60 year old man has a swollen knee. It is uncomfortable to lean on but not painful. Its been drianed in the past but the doctor is reluctant to drain as he says the risk of infection may be high. It is also accompanied by arm pain.

Any help for reducing the swelling or causes?
