I am 23 year old white male 5'10 ~170lbs. Lived in South Central United States for most of my life. Recently (about a year ago) I developed a sinus infection, which at the time didn't seem unusual because of the spring season change. The only known allergy that I have is anaphylaxis to insect stings (i.e. bee, wasp, etc). Naturally I went to the doctor and he prescribed Amoxocillin 500mg P.O. q.d. BID. After finishing the antibiotic, symptoms dissipated for about a week and then it started again. So back to the doctor again. This time we tried Clarithromyacin 500mg P.O. q.d.BID & Prednisone 20mg PO BID x6d -> 20mg PO qd x6d (YES that is correct 40mg A DAY! Might I add how much I hate steroids). After tweaking for a week or two, my body finally adjusted to the steroids and felt much better. Unfortunately about two weeks later.....same thing ALL OVER AGAIN! This has been a vicious cycle for the past year and for some reason I can't seem to find a physician who went to school for 8+years and has practiced medicine for 10+ years that can tell me why I am producing so much MUCUS. However let me give you the specifics:
Morning (~8am): Congested, mucus loosened in chest. Cough up GOURDS of Phlegm. Which I am positive are Post-Nasal Drainage. What is strange about it, is its consistency and color (although I'm aware these factors are irrelevant when making a diagnosis). It is VERY THICK and Paste like, similar to flour and water. However, after a HOT shower, my sinuses clear ever so slightly, and remain that way the rest of the day.
Evening (~8pm): Throat is dry from consistently clearing, coughing, and trying to blow what feels like OUNCES of mucus from my sinuses. But of course nothing comes out.
1. Adderrall 30mg ER PO
2. Animal Pak (Multi-vitamin)
3. Astelin Nasal Spray
1. Guafenisin 1200mg
2. Singular 10mg
3. Benadryl 50mg (can’t sleep w/o it because my breathing is aggravated)
4. Melatonin 3mg (to help me stay asleep)
5. Ibuprofen 400mg (to avoid the “hang-over” the Benadryl gives me the next morning)
Semi-regular…typical college student diet, eat when I get hungry and usually about the same time every day. Can be anything from fast food to home cooked meals. I do however, drink many caffeinated drinks to help me get through the day. (And I already know what you’ll say to that….but no it’s not the cause). I played a game of process of elimination last year to maybe pinpoint what was causing this…3months with NO CAFFIENE and still the same symptoms and only A LOT more fatigued. So….stuff needs to be done and it’s not going to wait for me to get well so…..I do what I can.
It has now gotten to the point where I’ve given up on feeling or breathing normal again. I don’t have days when I’m not sick, just good days and bad days. My doctors all say it is allergies and what they say correlate with my test results……but HOW? What could I have possibly become ALLERGIC to that I’ve not been exposed to MY ENTIRE LIFE? And why aren’t ANY of the allergy medications WORKING? Steroids just make my heart feel like it’s going to explode and the meds just make me a zombie. The other interesting thing about my “medical evaluations” is that NONE of the 9 doctors I visited requested or obtained a specimen for culture EVEN KNOWNING THE DURATION OF THIS CHRONIC ISSUE. And not one of them even preformed any type of procedure that would implement even the slightest clue of what is ACTUALLY up there. (i.e. nasal endoscopy, scope etc) However, I did reject a CT scan only because it is a $1000+ procedure and well…even prostitution wouldn’t pull that kind of cash for me and my insurance surely won’t pay it. But for good reason, I rejected it because there HAS GOT to be a way to diagnose this without exposing my head to radiation. I work in the laboratory, so I know there are Immunoglobulin studies that could be done, like IL-13 and IL-5….which Mayo clinic studies have shown that patients with Chronic Sinusitis have elevated levels of these due to MBP released from Eosinophils during destruction of normal air mold, which then cause tissue damage and result in secondary bacterial infections and the cycle repeats. Test like these, along with identifiable cultures and sensitivity studies could SURELY see if it is pathogenic or physiological. However the case, I’m tired of being ignored and tired of being well sick and tired. Somebody has got to know something about this. Below is my most recent CBC results:
WBC= 6.6 (10^3/μL)
NE% = 51.6
MO%= 9.2 (This is relative normal value for me….been high all my life)
EO%=6.1 (stayed at this or higher since 7/17/09 which was 1.6 then)
Also, I will add that I personally did a culture on a *STERILE* specimen I collected, I do understand the importance of cross-contamination ESPECIALLY with Microbiology. Anyhow, the results of those were actually surprising to even me. Not even normal flora.
