Severe pain & numbness in both toes

have had severe numbness & burning in both big toes for over 5 years. This occurs only when feet are elevated. I can walk usually with little or no pain. But even driving a car causes numbness. I have had every test that my doctors can think of with no conclusive diagnosis except "neropathology" (somewhere). Am know facing the only treatment left - a spinal "pacemaker" type implant.

Any suggestions would be most welcome. The pain is becoming almost unbearable at times.
