PCOS and swollen organs

My neice is 15 she has had a swollen spleen and pancerace for 2 years. Has not has a period for over 1 year. She has just been diagnosed with PCOS. That explains the symptoms of her skin, weight, and period but not the swollen organs. I know that PCOS is possibly caused by elevated inslin levels so could the swollen organs be related to the PCOS or is another disease causing both problems?
