Pain and sweeling in right knee

I have an ongoing condition that I would like some advise about. The symptoms include
Pain in right knee
weakness in knee and leg that is so bad I cannot get up from a squat without holding on to something for support
The knee stiffens up when driving or riding and will then pop and become extremely painful
There is some swelling but not a huge amount
I have had arthroscopic surgery on this knee twice and it has never been helped a lot by the surgery
Medical history
I am 57 years young, good health otherwise except for an ear issue
Female 6'4" tall 240 lbs but mostly muscle not hugely obese. I am active (well until recently because of the knee) love to dance, hike, swim....played basketball when I was younger which is part of the problems..
I take ibuprophin for this and alternate with aleve. I have tried ice packs, heat, hot tubs, whirlpools. Today I had a cortisone shot in the knee cap area.
I also got a prescription for voltarin.

I take condrointin and glucosimine
any ideas???
