Itching forearms

I have had this condition for many years having been told I have nodular purigo.
The intense intermittent burning, pin ****k itching sensation in the exact same place on both arms and shoulders but itching is more intense on upper fore arms. I have hard lumps of skin that often itch intensely and other areas no visible signs until I scratch. The condition totally dissapeared when I had chemo for breast cancer 3 yrs ago but came back!
It also is much better with exposure to the sun and worsens in the winter especially if arms are covered. At night sometimes the itching can keep me awake and a cold flannel only gives temporary relief.
I have been using fucibet for many years that does appear to give some relief to the itching but not always.
The main thing that strikes me as being strange is that the areas of itching, the leasions are at exactly the same place on each arm and shoulder area.
My sister and brother have very similar condition although one other sister has nothing.
I have done some research and have some ideas as to the condition but would be interested in some one elses opinion or ideas on this matter.
