Irregular flow during a period?

Usually I have very heavy periods, and they've been very regular since I first got it when I was 12 (I'm 17 now). However, for the past couple of months, the flow during my period has been really irregular. For the first two days, the flow is very heavy, as it usually has been. However, after that, the flow stops almost completely for about three days. After this, the flow is light to medium for three or four days until it stops completely for the month. Why is this happening? It's almost like having two separate periods per month. What can I do to get my cycle back on track?
