hormone problems?

Hi my probs have been developing slowly over 4 years now, and im now 20. The symptoms are getting increasingly worse.
STARTED WITH: fainting spells
vey low BMI 15 (not anarxic)
hands cold
2 YEAR LATER: increasing anxiety (over simple things)
increased frequency of urination (? anxiety related)
BMI 15
feel more tired from little exertion
hands cold
1 YEAR LARTER: anxiety still
BMI16 only just
lack stamina and energy
persistant worsening tiredness
feel weaker
hair is thinner and comes out
hands cold
dangerously low blood sugars (esp before food in morn)
cant gain weight (and i eat well and healtherly)
dizzy spells
fast heart rate at times around 130
What does this sound like? does it sound hormonal? Please post back. thanks
