High blood sugar and the doctors doesn't help .

Hello everyone ,
My father have type 2 diabetes and recently it got out f control , I took him to the doctor and he prescribed for him 2 types of Insulin
Lantus (20 units/100 each day) and Mixtard 30/70 (15/100 units with breakfast and 30 units with lunch and 10 units with dinner)
also he instructed him to take 2 pills of "Glucovance 500mg/5mgmetformin and glyburide) with every meal.
5 days ago he started to urinate many times a day (more than 10 times a day !) I started to monitor his blood sugar levels 3 days ago and its very high .
- A non-fasting was 221 .
- Fasting about after the lunch with 2:30 hours was 274 .
- A random test before the lunch (with a skipped dose of Mixtard with breakfast) it was 170 .
I found out that he is skipping the breakfast dose of Mixtard but is on the schedule of all the other doses yesterday I gave him the breakfast dose my self and made the test after the breakfast with 2 hours it was 269 .
I don't seem to understand what is the reason of this high results of blood sugar and I have visited the doctor again and he said that is normal !!
My father have a normal diet with no more than 150 carbohydrate in a single meal and most often he skips the dinner and ofcourse the dose with it.
I am currently in Egypt and the doctors here are horrible , they don't care for their patient or even bother to look into matters thoroughly .
I would really appreciate any advice or insights on how to control his diabetes .
