Help! hemiparesis right side with no found cause

10 years ago i began experiencing tricep weakness and hand weakness as well. I now have noticeable atrophy in upper arm, forearm as well as right hand. (you can actually visibly see the muscles are smaller/weaker in shoulder)

Now, my right foot and ankle/lower calf are smaller and weaker. There is a fair amount of twitching in the areas of muscle atrophy, my lower leg 'buzzes' on and off.

I do have large port wine stains in the areas of atrophy on upper body. (Neck upper right back/shoulder and down the tricep area, and a small one on wrist near thumb where there is also atrophy) No port wine stains on lower leg.

You can visibly see there is hemiparesis or muscle wasting/weakness mild on chest above breast and abs.

99% of all twitching, atrophy, symptoms etc are on the right side of body.

I have had spine and neck MRI, contrast and non contrast MRI of brain all relatively negative (other than C56, C6-7 radiculopathy)
might account for some of the tricep issue...
They have ruled out ALS and MS and stroke. Brain is normal.

HELP! I have had 3 different Neurologists say "ah its pretty slow don't worry about it". "We just dont know why"!
That is not good enough for me especially if we can help it with meds or therapy.
