Constipation, maybe?

For the last two days I have been a bit constipated and going to the toilet frequently to try to have a #2, but I can't properly - sometimes just small pieces come out and I still feel like I need the toilet, but I can't go. It's really starting to hurt my anus, because its sore and there was a little dot of blood on the tissue when I wiped - I think that's because of all the attempted bowel movements and wiping throughout the day.

What could this be?
What can I get from the pharmacy to treat this?

Maybe useful info:
Age 16
Low/Moderate daily exersize
Okay died, not too healthy though
Have decreased exersize and diet has got worse during the last few weeks because of the holidays and parties.

I REALLY NEED THIS SORTED. I am starting college on monday and the last thing I want is to be going for a dump every 5 minutes. Thanks.
