male patient aged 40 years has difficulty in speakin from past 2 years... MRI revealed anterior horn cell disease. But now since past 6 months his proximal muscles of hand are getting atrophied... he also has difficulty in arising from sitting on floor... there is hypotony of muscles of forearm....
