Bleeding during no.2!

I'm 18 yrs old and I have had IBS for a while, but it mainly use to be "the runs". Now i can hardly seem to get to go, despite eating a lot of fruit, veg, wholegrains etc. The past few times i have been, i have had to strain and so experienced a sharp pain and noticed bleeding. Could this be a fissure (split)? I've tried milk of magnesium but that wasnt much help as it upsets my stomach. I also get muscle pain in that region after i have had a bleed! Any advice? Thanks!!
