Back pain when swallowing

Lately I have been feeling strong back pain every time I swallow, particularly if my head is bent forward. The pain seems to radiate from my upper back to the right shoulder blade.
Other actions that aggravate the pain are singing high notes (or speaking in the higher register) and touching my Adam's apple. Even slight touch of the Adam's apple causes strong pain in the mentioned area.
Any ideas as to what might be going on here?
