Ab Pain for 3+ yrs

I have been experiencing lower ab pain for 3yrs with no diagnosis. I have had ovarian cysts but they come and go. No signs of endometrious, appendix removed, colon clear, physical therpy did not help, metformin, muscle relaxers, and bcs did not help.

Other than the pain, I exercise regularly and eat healthy. My core is weak. The pain is moderate to intense and pain killers don't touch the pain. I sleep with a heating pad to sooth the pain but nothing helps. Stress intensifes the pain as well as weight lost. I don't have much of an appetite. Pain wakes me in the night. Pain is spreading to lower back.

Any suggestions? I am planning to see a urologist next but I am not sure what to do. I need relief.
