Help! weird pains and breathing

My partner has been experiencing episodes for about 8 months. He gets chest pains, joint pains, fevers (feels freezing but is sweating alot).

He gets ill at about 3-6pm and it is usually gome by midnight. The next day he feels 'drunk'.

Sometimes he wakes at night with severe chest pain.

The paramedics could find nothing - ecg, blood sugars etc.

The doctor has done bloods, chest x-ray, sputum samples with no joy. The specialist is now insisting it is due to smoking.

Any ideas welcome as we are getting married in kenya soon and I am worried about the yellow fever vaccination.
