What is this?

I get severe cramping under my right breast right on top of my top rib. When I have a severe spasm my skin actually pulls up into a tight ball that you can see. It will stay like that for a few seconds and then when it releases you can actually see my skin drop. This happened for the first time in my gynecologists office right after I had my first child, 18 years ago. The doctor actually laughed, telling me he's never seen anything like that before. It happened right after I was laying down on the table and I tried to get back up. He never did anything about it. Ever since then it happens quite often. Does anyone know what this could be?
