PCOS and Organ Enlargement

My neice is 15 and has had spleen and pancrease swelling for over 2 years. She also was just diagnosed with PCOS. This explains her missed periods, her skin, and her weight gain but it does not explain the enlarged or swollen organs. I know that PCOS is thought to be caused by inslin resintance or excess inslin. Can the swollen organs be caused by excess inslin or could they be the cause of the inslin. I read about a tumor called an inslinoma. Not sure what to think.
