Intermittent noise in ear, sharp pain

Yesterday my left ear started making intermittent noises - sort of crackling/paper wobbling noises - a bit like loudspeaker noise when the rheostat connections aren't right. I have assumed that it is something happening on the ear drum itself.

Now they are coming every minute or so, accompanied by quite sharp, short term stabbing pains.
Sorry - a poor description I know, but I am in the middle of Argentina, I can't see a doctor easily, and I could do with some advice.

Anyone know what this might be?
