Ear pain with movement of head

I have had intermittent severe episodic left ear pain over the last 5-10 years. It comes on sporadically and feels like some type of muscle spasm. It gets much worse when I move my head as it feels like I am going to pull something loose right where my eardrum would be. It goes away after about 5 minutes spontaneously. Does anyone have any idea what is causing this? No fevers, tinnitus, decrease in hearing or vertigo.
