blood in saliva

I have been seeing blood in saliva often in late evenings. It happens for couple of mins and all normal after that. I am having this problem for ~1 year. Earlier it used to happen few times in a month. These days it happens 2-3 times a week.

Sometimes I notice blood coming from a gum in upper jaw. Most of the times the source is unknown. That is what worries me. I am 35 yrs old light smoker (7-8 sticks per day). I dont eat much spicy food and consume 2 ltr of water.

I have a cavity in wisdom tooth. no pain though.

Another symptom is pain in shoulder, elbow and ignoreable ache in left side of neck. So many of them not sure how to relate them. Had a x-ray of shouler done which was ok.

I have seen ENT specialist, he did nasalscope test and said bit sore throat and recommended antibiotics for 2 weeks. I had health screening done in last month. chest X-day, blood test, ECG stress test was all normal.

Any ideas what could be causing these and whom should I see next.
