small area of skin causing great pain at anus

I have a small area of skin causing great pain at anus (on the outside). It is at back of anus and makes cleaning difficult and that causes irritation. I have been to proctologist and had colonoscopy and he has not been able to help me. I have also been go GI doctor and treated for hemmorhoids. It just feels like normal skin but it is in the way (so to speak). It stays very sensitive too and sitting for long periods is difficult. I have had several MRI's to rule out spinal problems or coccyx problems.
